Welcome to the Grad Night Registration Form

Please note the following:

• A valid email is required – once completed a copy of this form will be emailed to the address entered
• Required Parent and Student Agreements must be uploaded here or turned into the office fully filled out

  (Some cell phones do not work well with fillable forms – if having difficulty please either use a laptop or print the form and have your student take it into the office in an envelope. Whether on a cell phone or a laptop the forms must be saved to your file system before they can be filled out and uploaded.)
• Payment can be below using PayPal or Cash/Check.
  Checks: Make payable to El Modena Grad Night (write Student Name and ID Number in memo)
• A Grad Night Drop Box is located in school office – Forms or envelopes with CASH/Checks must include Student Name and ID Number – the committee needs to be able to match payment to registration.
