By signing this form, student/parents (or guardians) agree to and understand the following:
1. Transportation options will be discussed between students/parents. Both understand that El Modena Grad Night Committee strongly encourages that parents or other adult drop off and pick up students due to late-night and drowsy driving conditions that may be experienced if the student drives or rides with a fellow student. Further, students/parents are aware of legal restrictions on a student’s driver’s license regarding curfew and/or provisional license rules. Should the student/parent decide to have the student drive, both understand that Grad Night parking will be valet-style, and keys will be held for the duration of the event or until the student leaves following the procedure described below.
2. Students/parents understand that check-in is between 9 &10 p.m. for the 10 p.m. start of the event – Grad Night ends at 4:30 a.m., on Friday, June 9. The El Modena Grad Night Committee encourages all students to stay for the entire event. Both understand that if the student chooses to leave early, parents will be called to come pick up the student or give permission for the student to leave the event. Further, the El Modena Grad Night Committee is not responsible for the student, or any actions the student may engage in, should the student leave Grad Night early.
3. The student will not bring alcohol, tobacco or other drugs to Grad Night or come to the event under the influence of any chemical substance. Further, the student may not bring any medications (including aspirin, NoDoz, etc.) to Grad Night. Any necessary medications must be accompanied by a note from the parent.
4. Student/parent understand that if student is in possession of, or showing the effects of, a chemical substance, or behaving in such a way as to disrupt the event, parent will be called to pick up the student from Grad Night. If the parent is unable to pick up student, law enforcement will be called. Both also understand that any prohibited substance or item found in student’s possession will be confiscated and reported to law enforcement.
5. Parent will provide two phone numbers where parent can be reached during Grad Night, in case of an emergency or in the event student wants to leave early.
6.Student/parent understands that a Grad Night volunteer will do a “pat down” of each student attending. Both understand that El Modena Grad Night is not responsible for lost or stolen valuables.
7. Student/parent understand that student may be photographed during the event. Both give permission for photos to be used for promotional purposes by El Modena Grad Night.
8. Parent gives permission for student to participate in Grad Night.

Further, I am aware of legal restrictions on my driver’s license regarding curfew and/or provisional license rules. Should I decide to drive, I understand that Grad Night parking will be valet-style, and my keys will be held for the duration of the event or until I leave following the procedure described below.
